- In order to be able to pick up your equipment for football, you will need the following:
- A) Pop Warner Physical Form completed by a physician- (this is the ONLY acceptable physical form- school forms are not acceptable).
- B) 1 Wallet sized picture of face (no hats) - we only have a 2x2" space, do not send big pictures.
- C) 1 copy of Final report card - front & back (must have all semester/ quarters) Final, as in last report card of this year. Do not send incomplete report cards.
- D) Copy of original birth certificate (with state seal visible)
- E) Copy of health insurance card - front & back
- F) $150 equipment deposit check - this will not be cashed and will be returned to you at the end of the season. Please bring this with you when you pick up the equipment. The check should be made out to PBGYAA Tackle Football.
NO equipment will be issued unless you have submitted all of this information. You may not have the final/last report card of the 2019-2020 school year, so please be sure that you have ALL other paperwork and we will accept the final report card at the 2nd paperwork hand in.
